Please select the format the best suits your needs.
Step	1		To view and print a copy of the Registration Form in Adobe PDF format.
Step	1		To view and print a copy of the Registration Form in Microsoft Word format.
Step	2	Complete the form, recheck your choices and enclose the correct $$'s
Step	3		Mail your completed Registration Form to: DX2006 c/o Dave Shipman, VE7CFD 1013 Sinclair St. West Vancouver, BC Canada V7V 3W1
Step	4	Approximately 10 days after you mail your Registration Form, you can check the Registered Guest listing to see that your registration has been processed.
